Five Whys

The Five Whys technique is a problem-solving method used to explore the underlying causes of a particular issue by repeatedly asking the question "Why?" This iterative interrogation helps peel back the layers of symptoms to reach the root cause of a problem.

IMPLEMENTATION

  1. Identify the Problem: Start with a clear and concise statement of the issue at hand.

  2. Ask the First Why: Pose the question "Why?" to the initial problem statement to uncover the first layer of reasons.

  3. Continue the Process: After each answer, ask "Why?" again to delve deeper into the subsequent layers of explanation, aiming for five levels of questioning or until a root cause is clearly identified.

  4. Document Each Step: Keep a record of each question and answer to track the reasoning process and ensure every step is logically connected.

  5. Identify Solutions: Once the root cause is identified, brainstorm potential solutions that address this fundamental issue, not just its symptoms.
